Nirvana Wellness Medspa Gift Card Terms

• Gift cards do not expire.

• After 12 months, a $3.00 monthly service fee will be deducted from the remaining balance.

• Cards can be used for any service or product at Nirvana Wellness Medspa.

• Gift cards are non-refundable and not redeemable for cash (except when the remaining balance is $5.00 or less, per PA law).

• Treat gift cards like cash; lost or stolen cards cannot be replaced without proof of purchase.

• Unused balances may be reported to the Commonwealth of Pennsylvania after 5 years of inactivity.
